Home health has become an important piece of the full spectrum of care provided by Franciscan Health. Home Health Directors are the bridge between the physician and the patients. This role oversees the hands-on care of our patients – ensuring that each individual receives compassionate care in line with our expectations and in compliance with all regulations. You have to be good at managing staff, preparing budgets, leading a team… key management skills that all contribute to the highest quality home healthcare.

WHAT YOU WILL DO
Plan and direct operations to ensure the provision of adequate and appropriate care and services.
Implement Governing Body directives and organizational Policies and Procedures.
Contribute towards successful implementation of annual operating plan and assists with financial analysis, and formulation of recommendations to achieve plan objectives.
Develop, review, implement, and evaluate policies and procedures.
Determine the need for contract services; develops contracts for negotiation; assures compliance with contracts.
Coordinate with related programs and services.
Plan, develop, implement and evaluate agency programs.
Inform the Governing Body and staff of current organizational, community, and industry trends.
Ensure adequate staff education.
Maintain an employee feedback process to enhance work processes and employee understanding.
Ensure that appropriate staff supervision and support exists.
Plan, budget and manage the fiscal operations of the agency.
Manage operations in accordance with established fiscal parameters.
Direct the PI Coordinator to ensure that PI activities effectively meet agency needs and monitors the design and progress of PI programs.
Evaluate services and programs provided on an ongoing basis and through the performance improvement and outcomes-based quality improvement processes.
Provide and support the participation of staff in applying the principle/model of continuous improvement to improve existing processes and enhance the effective utilization of resources (human and material).
Administer preparation of reports, documents, statistical surveys, concerns/issues and accomplishments for submission.
WHAT IT TAKES TO SUCCEED
Assess and interpret data about a patient’s status in order to identify each patient’s age/development specific needs and provide the care needed by the patient’s age group.
Comprehensive understanding of Hospice philosophy and operations.
Healthcare administrative or program management experience.
Excellent interpersonal skills necessary in order to interface effectively with all constituency groups and colleagues.

3-5 Years Experience
License(s): Registered Nurse
Certification(s): Basic Life Support Program
Travel: Up to 20%
