Recovery is seen as a personal journey that utilizes supportive relationships, empowerment, social inclusion, and coping skills.Position Responsibilities: The Advanced Practice Nurse (APN) will function in a lead role in the planning efforts toward integrating primary care and behavioral health care to identified clients of the Community Mental Health Center. Qualifications:
Masters degree in nursing with a major in family practice Licensed by the State of Indiana Health Professions Bureau as a Registered Nurse with Prescriptive Authority or eligible to apply for prescriptive authority is required for the position.
Minimum 3 years experience required.
Experience in a primary care setting and with persons with serious mental illness.
Experience working with adolescent/children with mental health illnesses is preferred.
Day to Day Expectations:
Coordinates planning activities associated with the planning/ implementation grant projects.
Provides initial and on-going medical and psychiatric assessments for clients who are served by the integrated primary care service in collaboration with a physician.
Provides administrative and clinical supervision to staff assigned to the integrated health care program.
Participates in the Center's Staff Development Programs by fulfilling identified items on the Individual Employee Development Plan.
Completes appropriate clinical documentation to ensure both billing and the medication record are complete and accurate.
