Flexible. Traditional M-F 8a-5p, four (10 hour shifts) or other combination to expand hours for patient access.

Job Location

This nurse practitioner or physician assistant will join Community Health Network, a multispecialty group with over 1,100 providers in Indiana. This internal medicine opportunity will be located at one of our newly acquired and renovated practices: Internal Medicine Corwin Lane, Kokomo, IN.

Highlights

Community Health Network has an excellent opportunity for a board certified, internal medicine or family medicine trained advanced practice provider to work at our outpatient Internal Medicine Care practice, a part of our Community Howard Regional Group: the healthcare provider of choice in North Central Indiana.

• Full-time position with competitive compensation and benefit package

• Flexible scheduling options: Hours and days are variable subject to provider’s preference and availability (extended hours option: four 10’s or three 12’s)

• Clinic is open Monday-Friday, no weekends

• Autonomy in providing patient care. Busy office with high patient volume

• Opportunity to establish lasting rapport and patient relationships

• Electronic Medical Record (EPIC)

Experience

Family Medicine trained and board-certified nurse practitioner or physician assistant. Open to new graduates.

Job Duties

Nurse practitioner or physician assistant will provide healthcare, including assessment, diagnosis, and treatment of acute and chronic health conditions and wellness preventative medicine. Promotes a healthy lifestyle and involves the client in his/her plan of care with emphasis placed on patient self-determination and maximization of client knowledge to enhance care of oneself.

Education

Educational background consistent with requirements for licensure and certification with the State of Indiana. Must successfully complete a nationally recognized Advanced Practice Nurse certification examination or have current national certification; must maintain certification on an ongoing basis. Must be eligible for prescriptive authority under the criteria outlined by the Indiana State Board of Nursing; must maintain eligibility for prescriptive authority on an ongoing basis.
