Job LocationThe Transitional Care Coordinator - LPN position is located in Indianapolis, Indiana. This LPN will sit at Community Hospital East, Indianapolis, IN.Job Duties
The Transitional Care Coordinator – LPN follows predetermined high risk for readmission patients after discharge using an evidence based care coordination approach.
Coordinates with a multidisciplinary team in order to provide services that will help interrupt patterns of frequent acute hospital or emergency department use and health status decline.
Supports the objective and goals of Accountable Care.
ExperienceExperience in medical office or healthcare setting preferred.EducationGraduate of a recognized accredited school of practical nursing, or Graduate from a non-accredited school of practical nursing with three years related professional nursing experience.LicenseValid Indiana Licensed Practical Nurse license.Skills
Excellent oral and written communication skills.
Flexibility for ever changing priorities.
Comfort with an independent role.
Understanding of the healthcare system and its component parts, including sites of care, delivery models, and the role of various providers and healthcare professionals, as well as, complex care positions and roles.
Demonstrated ability in handling patient confidentiality.
Ability to work with peers in a team situation.
Ability to collaborate and facilitate relationships and consensus among the patient, family, and multidisciplinary providers from many different disciplines.
