Job Location The Case Manager position is located in Indianapolis, Indiana.Job DutiesThe Case Manager is responsible for the assessment, planning, implementation, coordination, monitoring, and evaluation of services across the continuum of care to ensure quality patient outcomes and appropriate utilization of health care services.The Case Manager is responsible for supporting the healthcare team towards a smooth transition from one level of care to another in support of the patient/family.ExperienceMaster’s Degree with three years of recent clinical experience preferred.EducationMaster’s Degree in Social Work required.LicenseCurrently licensed by the State of Indiana as a Social Worker required (or contingent of obtaining license within 30 days of current employment); Certified Case Manager (CCM) or ACM preferred.Skills

  • Excellent communication, interpersonal, strong organizational, problem solving, decision making, and technical skills

  • Ability to establish rapport with a wide variety of people

  • Exhibit openness to change and willingness to try new approaches to problem solving
