The RN Care Manager will work collaboratively with the multidisciplinary team to manage catastrophic cases and deliver top quality care management services to identified patient populations. Advocates, guides, and intervenes on behalf of patients, families, and/or caregivers. Engages identified patients in appropriate care management programs and provides patient centered interventions. Strives to positively influence the overall patient health outcomes as measured by clinical quality and utilization measures. Supports the achievement of system-wide population health goals within the framework of continuous improvement methodologies.
Associate's Degree in Nursing required; BSN preferred.
Requires current state of Indiana license as a Registered Nurse.
3-5 years of relevant experience required.
Critical care nursing experience preferred to include end-of-life care and advanced planning discussions.
Certification in Care Management preferred.
Strong professional level knowledge of comprehensive clinical assessment skills preferred.
Knowledge of Medicare/Medicaid, insurance and regulatory guidelines is preferred.
