Fairbanks is located on Community North’s campus in Indianapolis, Indiana.

Job Duties

The Care Coordinator role will service our admissions department. The role will consist of two main portions. One main portion would be working in the call center accepting incoming patients, families, and referrals. The other main portion of the role would be to handle the front end admissions. The Care Coordinator will funciton similiar to a case manager.


Bachelor’s Degree plus (2) years’ experience in mental health related field preferred


Bachelor’s degree in mental health related field required


  • Must have the ability to relate in a non-judgmental and culturally competent manner

  • Must have interpersonal skills including motivation, flexibility, persistence, and patience

  • Must value client/family centered care and able to apply strengths based needs assessment

  • Must have the knowledge of behavioral health competencies specific to population being served

  • Must possess the ability to function independently under supervision, problem solving abilities/good judgment
