Description

The Call Center staff provides support to skilled nursing centers by clinically assessing referrals, verifying benefits and communicating between liaisons and the referring hospital. The Call Center staff must have a sharp memory, and be able to multi-task effectivity with coworkers in a fast-paced environment. The hours are 12:00 noon to 9:00 pm. M-F.

  • Clinically reviews incoming referrals for post-acute skilled patients through Electronic Referral Systems

  • Verifies benefit of payment - knowledge of reimbursement for Medicare, Medicaid and other payor sources.

  • Facilitates the involvement of Clinical Liaison and the Care Centers to qualify the referral for admission.

  • Demonstrates efficient and effective management of the referral.

Requirements

Must adhere to the Company’s Code of Conduct and Business Ethics policy including documentation and reporting responsibilities.

  • Minimum two (2) years of clinical care setting or related health care experience.Minimum of one (1) year of experience in a long-term care setting as an MDS/Admissions Coordinator/Case Manager is preferred.

  • Demonstrates clinical assessment skills at the level necessary to meet the job requirements.

  • Ability to make independent decisions on a regular basis.

  • Strong computer skills with duel screen experience

  • Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.

  • Able to relate positively, favorably and cooperatively with others.

  • Must be capable of maintaining regular attendance.

  • Effective interpersonal skills and the ability to work with the office team.

  • Strong understanding of computer technology, including email use.

  • Must be able to maintain confidentiality regarding patient, employee and company proprietary information

  • Must have the ability to relate professionally and positively and work cooperatively with patients, families, and other employees at all levels

  • Mobility, reaching, bending, lifting (minimum of 10 pounds), fine hand coordination, ability to read and write, the ability to understand and follow written and oral instructions and directions, ability to travel, and ability to remain calm under pressure
