Triage Specialist assists in the coordination and care of patients seeking behavioral health services. The essential function is to determine the nature and severity of the mental health problem of patients presenting to the Crisis Department and to give report to the Crisis Therapists regarding these patients. Triage patients for risk and acuity and document this triage in EMR. Take patients’ vital signs in coordination with Crisis RN. Document pertinent collateral information obtained from outside resources/referrals. Give case overview and hand-off to Licensed Clinical professional/Crisis Therapist. Assist with monitoring patients and safety in the Crisis area. Assist crisis therapists with admission process as needed. Assist with completing precertification of a psychiatric admission as needed. Assist with reviewing clinical information for direct admission requests as needed. Answer calls to 24/7 Crisis line. Initiate telephone triage and transfer clinical calls to appropriate service areas.
High School Diploma or GED (Required)
4 year / Bachelor's Degree : Bachelor’s degree in Human Service related field preferred. (Preferred)
