Parkview North Hospital POHCI Care Navigator in Fort Wayne, Indiana
The POHCI Care Navigator provides services for complex Parkview Occupational Health patients in order to improve coordination of care, facilitate additional testing and coordinate results delivery in an effort to maximize the overall patient care experience. Serves as a point of contact for any company representatives, Business Development Consultants, patients or other key stakeholders. Makes patient contact starting with generation of the referral and managing the entire patient experience through to its conclusion. Coordinates patient experience and develops a “partnership” type relationship that ensures patient understanding of the testing and/or examination, follow up requirements and documentation with the goal of increasing patient compliance and satisfaction. Provides timely follow up with patients that are scheduled with the goal of successfully coordinating physical examinations and/or testing. Acts as a liaison between the patient, company representative, provider and clinic to coordinate scheduling and deliver results in a timely manner. Tracks results and interpretation progress to be sure that documentation is delivered in a timely manner as directed by the provider. Follows up as necessary. Ensures that the referring physician receives interpreted study results in a timely manner.
High School graduate or GED. Associate's degree in healthcare related field preferred.
Prefer certification as a Medical Assistant.
Minimum one year experience working in a health care facility with direct patient care.
