Parkview Health Occupational Therapist - HH/H in Fort Wayne, Indiana

Summary: Evaluates patient's condition and/or disability, by selecting and administering appropriate tests and measures; formulates and administers a treatment plan; interprets testing and treatment to staff, patient, family; determines need for referral to other specialists and/or agencies; maintains proper records and reports.

Education: Must have a Bachelor's or Master's degree in Occupational Therapy.

Licensure/Certification: Must hold or be eligible to receive an Occupational Therapy license in the State of Indiana, from the Indiana Professional Licensure Board. Must have a valid state driver's license, reliable transportation, and proof of insurance. Must have current CPR.

Experience: At least one year experience in a clinical setting related to the departmental scope of service provided. In lieu of previous work experience, verification of supervised clinical experience in Occupational Therapy.

Other Qualifications: Must be willing to travel within the workday and be willing to make home visits. Must be able to lift 50 lbs.
