Parkview Health Medical Social Worker II - Sign-On Bonus in Fort Wayne, Indiana

Summary: Social Worker assesses patient/family needs in order to effect change in behavior and provide skill building/psycho-educational interventions as requested. Provides assistance to the multidisciplinary team related to placement, discharge planning, treatment planning, linkage to community resources and crisis intervention. Coordinates with case management coworkers to facilitate the achievement of quality and cost effective patient outcomes. Provides discharge planning and appropriate linkage to community resources

Education: Master's degree from a school of social work and one (1) years of social work experience in a health care setting.

Licensure/Certification: Must have a current licensure by the State of Indiana Health Professions Bureau as a LCWS/LSW. Must have a valid state driver's license, reliable transportation, and proof of auto insurance.

Experience: Minimum of one (1) years' experience in health care with experience in home health care preferred.

Other Qualifications: Demonstrates thorough working knowledge of social work process, community agencies, services, and resources available to assist patients and families in the resolution of emotional, social, and financial difficulties related to change in patient's health status. Requires general knowledge of the normal growth, development, related behaviors, needs, comfort, and range of treatment appropriate to the age of the patient and diagnosis to support the provision of an individualized plan of care. Requires sound analytical and organizational skill to assess patient's needs, plan and coordinate appropriate therapeutic intervention and to manage case load to achieve efficiency and productivity. Must have good verbal and written communication skill.
