Where you fit in

This position is primarily focused on doing admissions and providing emotional support for families. The Home Care Registered Nurse is responsible for case management of a group of home health care clients, providing direct nursing care, and coordinating and supervising other disciplines involved to promote positive health outcomes and quality services to patients. Responsible for providing skilled care to assigned patient according to the plan of care established by the patient’s physician. Practices within the scope of nursing as defined by the Nurse Practice Act.

Your exceptional qualifications

Graduate of National League for Nursing (NLN), Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA) accredited school of nursing or three years of related professional nursing experience.

Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC).

Bachelor of Science in Nursing preferred.

One year nursing experience required.

Home care/hospice experience preferred.

CPR certified
