Who You’ll Be Working With:

As a member of a team of nurses, social workers, registered dieticians, physicians, pharmacists, health economists, and program coordinators, you will have the opportunity to make a profound impact on the lives of people with various types of insurance who have medical, behavioral and/or substance abuse issues. You will serve as a critical link to connect patients in the community with case management services.

What You’ll Be Doing:

  • Make telephonic outreach to the “yet-to-be-reached” and “lost-to-contact” patients.

  • Utilize a variety of outreach strategies to engage various populations.

  • Identify the gaps in use of services such as physician access through discussion with patients.

  • Educate and inform patients on the services/programs offered.

  • Identify patients’ willingness to access and enroll in the program.

  • Identify and address the barriers associated with accessing the program.

  • Utilize engagement strategies to build rapport.

  • Works closely with the clinical team to support patient engagement and confirm the patient has connected with resources, as appropriate.

  • Contact with provider offices and other clinical settings to obtain clinical data and/or patient demographics to facilitate care.

  • Receiving referrals from community providers through Infiina and implementing outreach and documentation process.

  • Receive and upload clinical data sent through Infina to be included in Identifi patient record.

  • Communicate patient care plans and clinical updates to providers receiving this information through Infina.

  • Communicate with nursing staff for situations that require nursing judgment.

  • Other duties as required.

The Experience You Need (Required):

  • 3-5 years of experience in healthcare, preferably in a setting with patient contact and outreach efforts, service-oriented.

Finishing Touches (Preferred):

  • Healthcare-related bachelor’s degree.

  • Bilingual English and Spanish.

  • Ability to travel to various locations.

  • Must demonstrate excellent interpersonal communication skills, including exceptional listening skills.

  • Must have the ability to positively influence others with respect and compassion.

  • Must demonstrate intellectual independence and willingness to learn new skills.

  • Must be flexible and comfortable with ambiguity and change.

  • Experience working directly with patients, physicians, physician office staff, healthcare team, etc. telephonically (i.e. call center).

  • Experience with coordinating care, advocating, or navigating on behalf of patients.

  • Experience with Medicare, Medicaid, third-party insurance, copayments, etc.

  • Knowledge of telephonic translator services and how to use them.

  • Strong work ethic built on a foundation of proactivity and teamwork.

  • Ability to navigate ambiguity with the aid of structured problem solving techniques.

  • Computer proficiency in Microsoft Office.
